FAX # DEPENDS ON WHICH SW III TOOK THE REPORT, SW WILL PROVIDE # AT THE TIME OF THE CALL

TO SATISFY THE MANDATE, THE SUSPECTED CHILD ABUSE REPORT: MUST BE PHONED IN IMMEDIATELY AND THE WRITTEN REPORT MUST BE SENT VIA MAIL OR FAX (within 36 hours of your call)

The original SCAR may be retained for your records.
